- Certificate as Ayurved Nurse and Midwife Dhatri (from Ayurvedic and Unani Tibbi Medicine Board of UP)

- Online Registration: Candidates must complete One Time Registration (OTR) on the UPPSC portal. Registration will be completed only after 72 hours, so ensure this is done in advance.
